Consent Process

MAXIVITA LLC captures written SMS opt-in consent from each sales representative during the onboarding flow at portal.maxivita.io. Recipients see the consent checkbox unchecked by default and must affirmatively check it to opt in. We log the consent text shown, timestamp (UTC), IP address, user agent, and the phone number the consent applies to.

Consent text shown to reps: “I agree to receive operational SMS notifications from MAXIVITA LLC at the phone number provided as part of my onboarding. Message frequency varies. Message and data rates may apply. Reply STOP to opt out, HELP for help. See Privacy Policy at https://www.maxivitapestcontrol.com/privacy-policy.”

Recipients may opt out at any time by replying STOP. Our system honors opt-out at send time and refuses subsequent messages to opted-out numbers.
